Unlimited Care Inc. is looking for a Service Coordinator Assistant
for the Medicaid Waiver Participants in the Nursing Home Transition
and Diversion and Traumatic Brain Injury program in Kingston, NY.
Essential Job Functions: 1. Assist Service Coordinators in
developing comprehensive Service Plans, provision of services, and
scheduling participant medical visits and transportation requests
2. Handle phone inquiries, email and fax correspondence and take
necessary steps to forward messages or remediate service issues as
they arise 3. Maintain, organize, and assure appropriate filing of
all charts and reports in a timely manner, including annual
thinning of all charts 4. Attend Team Meetings and service plan
development meetings, as appropriate 5. Assist management with
audits of participant records in agency branches monthly, or as
requested Candidates must possess: · A bachelor’s degree in social
work, psychology, or closely related field · Proficiency with
Microsoft Suite · Team player able to work both independently and
in a group · Possess strong verbal and written communication skills
· Ability to work effectively in a fast paced, multi-tasked
environment · A valid NYS driver's license is required and ability
