Job Description Our Charlton Family Medicine Office in Ballston
Lake is looking for an RN to join their team! This is a Part Time
position. The hours are: Monday and Wednesday 8:00 am -5:00 pm and
Friday 8:00 am - 4:00 pm. \n This position is eligible for up to a
$3,500 recruitment and retention bonus \n Responsibilities: \n \n
Document verbal conversations with patients when appropriate. \n
Document information into patient record in an accurate, legible
and timely manner. \n Assess vital signs and the reason for each
patient visit. \n Triage patient phone calls: collect, analyze and
create a well-documented plan of care for the patient based on
their presenting symptoms and expected healthcare outcome. \n
Prepare patient for visit as appropriate, based on office/provider

Compensation: $32.00 - $40.00 per hour \n CCP salary ranges are
designed to be competitive with room for professional and financial
growth. Individual compensation is based on several factors unique
to each candidate, such as work experience, qualifications, and
