Job Description:
Albany Medical Center, a leading healthcare system, is seeking a
dedicated Clinical Nurse II for our E8 Rehabilitation Unit. This

Nurse II, you will apply specialized skills to help patients
recover physical function and improve quality of life after
significant medical events. Provide comprehensive and compassionate
nursing care to rehabilitation patients. Develop and implement
individualized care plans based on patient assessments. Monitor
patient progress and adapt plans as necessary. Educate patients and
their families on treatment plans and health management
post-discharge. Collaborate with a multidisciplinary team to ensure
the optimal patient care and outcomes. Maintain accurate patient
records and administer medications as prescribed. Strong clinical
skills in a rehabilitation setting Excellent patient assessment
capabilities Ability to manage care plans effectively Proficient in
using medical software for patient documentation Strong
communication skills for interacting with patients, families, and
team members Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Registered Nurse (RN) licensure in the state of
practice Certified Rehabilitation Registered Nurse (CRRN) preferred
but not required
