This position is responsible to work both in our primary care clinic (on-site at 1938 Curry Road in Rotterdam) and completing in-home patient visits in close proximity to our Center. Works as a key member of our primary care team (one physician and 2 nurse practitioners, RN Supervisor and LPN), along with our broader interdisciplinary team caring those enrolled in our PACE program. The goal of our program is to help individuals 55 years and over to remain in the community as long as possible, delaying or avoiding nursing home placement. Work schedule Monday to Friday 8am to 4:30pmRequirements:NYS RN Licensure.Min 1 year experience, preferably caring for seniors. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
