Job Description:
Department/Unit: Float Pool Work Shift: Night (United States of
America) POSITION SUMMARY:
The Licensed Practical Nurse works within the framework of the
Patient Care Services Organization as a member of
the health care team. The primary responsibilities of the Licensed
Practical Nurse is to deliver patient care under the
direction of a Registered Professional Nurse within the realm of
the New York State Practice Act.
QUALIFICATION REQUIREMENTS:
Assist patients in reaching outcomes as established by the RPN in
the plan of care.
The LPN gathers, and documents data related to physiologic,
cognitive, cultural, and spiritual status, assists in goal
directed plans of care which are evidence-based and age specific,
and implements care through utilization and
adherence to established standards which define the structure,
process, and desired patient outcomes of the nursing
process, and communicates information needing referral to RPN and
reinforces teaching plan established by RPN.
To perform this job successfully, an individual must be able to
perform each essential duty satisfactorily as
outlined in the attached annual competency assessment. Where

adheres to unit and human resource policies, participates in
development and attainment of unit/service,
and patient care goals, and organizes the delivery of patient care
in an efficient and cost effective manner.
The LPN assumes responsibility and accountability for professional
standards and for own professional
practice, attends Inservice Education Programs as required by unit
and hospital policy, maintains role
specific annual competency, recognizes and communicates situations
needing referral to an RPN, and
relates to/assists patients, families, significant others, and
coworkers with respect and kindness. The LPN
maintains confidentiality of information, demonstrates the
knowledge and skills necessary to provide
patient care appropriate to the age and special characteristics of
the patients served, and performs data
collection/documents interventions within protocol or plan of care
and objective patient care data on Daily
System Assessment.
The LPN may not initiate Plans of care or Protocols, complete 24
hour analysis or End of Shift Summaries, or give
end of shift report.
Provide technical support as demonstrated by knowledge base and
validation of competency skills related to:
Medication administration, assisting and providing care to clients
with peripheral IV Therapy only (no central line
devices), performing phlebotomy, safety measures, personal hygiene
skills, nutritional skills, elimination skills, heat
and cold applications, admission/transfers/discharge, perioperative
care, musculosketal skills, respiratory skills,
transfer/transport skills, and specimen collection/testing.
Assure Safety and Cleaniness of Environment, Equipment and Supplies
by providing a clean and safe
environment to meet quality patient care. He/she cleans up after
work is completed in common areas, monitors halls
and common areas to assure equipment is in its proper place, and
cleans equipment.
LPN III's Only may, under the direct supervision of a Registered
Professional Nurse monitor (not initiate) Blood
product administration, via peripheral routes only, initiate
Peripheral Intravenous Therapy , administer pre-medicated
IV fluids, and flush intermittant peripheral venous access devices
with normal saline solutions to maintain patency.
EDUCATION:
Must be a graduate of an accredited School of Practical Nursing.
Must hold current New York State registration or
possess a limited permit to practice in the State of New York and
current BLS certification.

Based on unit-specific criteria.

COMMUNICATION SKILLS:
Ability to read and interpret documents such as safety rules and
procedure manuals. Ability to document patient care
on established forms. Ability to communicate cooperatively and
effectively to patients, family members, employees
and others. Ability to listen well, to take direction and to engage
in interactive dialogues with others. Ability to seek
out the input of others to achieve consensus.
MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide
in all units of measure, using whole
numbers, common fractions, and decimals, accurately with medication
administration. Ability to compute rate, ratio,
and percent.
REASONING ABILITY: Ability to apply common sense understanding to
carry out instructions furnished in
written, oral, or diagram form. Ability to deal with problems
involving several concrete variables in standardized
situations.
PHYSICAL DEMANDS: The physical demands described here are
representative of those that must be met by an
employee to successfully perform the essential functions of this
job.While performing the duties of this job, the
employee is regularly required to stand, walk, use hands to probe,
handle, or feel objects, tools, or controls, reach
with hands and arms, and speak and hear. The employee is
occasionally required to sit and stoop, kneel, or crouch.
The employee must regularly lift and/or move up to 50 pounds and
frequently lift and/or move more than 50 pounds.
Specific vision abilities required by this job include close
vision, distance vision, peripheral vision, depth perception,
and the ability to adjust focus.
WORK ENVIRONMENT:
The work environment characteristics described here are
representative of those an employee encounters while
performing the essential functions of this job. While performing
the duties of this job, the employee may be subject
to infectious materials and chemicals (see unit specific MSDS
information).
The noise level in the work environment is usually moderate.
This job requires as an essential function that the majority of the
time the employee be physically on-site as the work
cannot be done from a remote location.
