Status: Full-time, Exempt
Summary:
Under the direction of the Leader of Case Management, the
Behavioral Health Case Manager will provide education and support
to customers with behavioral health conditions. The Behavioral
Health Case Manager will be responsible for improving health
outcomes of and promoting appropriate preventive care for MVP's
members with behavioral health conditions. MVP's Behavioral Health
Case Manager will coordinate care during hospitalizations and post
discharge, to assist our members in overcoming major barriers to
care. The goals of the Case Manager are to improve the mental
health and substance use disorder health outcomes of these at-risk
customers and to reduce MVP's overall readmission rates and improve
customers follow-up with behavioral health care. This role will
perform assessments, care planning, facilitation, care
coordination, evaluation, and advocacy for customers to promote
quality, cost-effective outcomes. Performs other duties as
required.
POSITION QUALIFICATIONS
Minimum Education
Master's prepared mental health clinician (LCSW, LMHC), or licensed
Registered Nurse (RN). New York State licensure required with
previous mental health or substance use experience. Case Management
Certification preferred (ANCC or CCM).
Minimum Experience:
Minimum of 2 years of recent mental health or substance use
experience required. Utilization or Case Management experience in a
managed care organization is preferred.
