Responsibilities of the Case Manager for the Behavioral Health Plus Program:Coordinating, integrating, overseeing and monitoring all aspects of the physical, behavioral and social health needs of the High-Need Seriously Mentally Ill (SMI) population.Duties include: Engage clients and assess eligibility for the High-Need SMI population.Complete enrollment and ongoing assessment process as appropriate. Provide weekly intensive care management services to high need clients by developing an interdisciplinary plan of care reflecting an increased intensity of needs and aimed at helping the client achieve stability within the community.Work closely with the individual and care providers to implement the intensive plan of care. Complete mental health assessments for Managed Medicaid clients as needed, including but not limited to eligibility for Home and Community Based Services. Provide referrals and linkage to community services as needed including: clinical, mental health/substance abuse treatment, medical/health services, legal services, community support, financial, transportation, etc. Provide comprehensive transitional care from inpatient to other settings to ensure clients are linked with necessary services upon discharge from an inpatient setting or a transition to another service provider. Regularly communicate with other care providers to coordinate the exchange of information and identify other care requirements and needs. Resolve long standing issues that affect smooth delivery of care, clients' stability in the community and health promotion. Evaluate outcomes of care with the clients' team by monitoring and tracking success of the clients' care plan goals. Monitor delivery of care via ongoing communication with client, providers, managed care companies and natural supports.Advocate for additional services as needed. Caseload average is 25 as the position will be providing intensive services to clients with SMI. The COVID-19 vaccine is required or properly approved exemption consistent with applicable laws. Visit our website at us on Facebook!Affirmative Action/EEORequirementsSkills/Qualifications for the Case Manager for the Behavioral Health Plus Program include: BA/BS in Human Services and two years' experience or a Master's degree with one year's experience; 1 year in case management (defined as experience in assessment of client's need, care planning, implementation of care plan and regular review along with benefit entitlements, linkage to community providers and services); Knowledge of mental illness/behavioral health, substance abuse, physical/medical health, and homelessness is required; A demonstrated ability to function as a member of an interdisciplinary team; 1 year experience working in a health care setting providing direct care to clients is preferred; Valid and insurable driver's licenses as agencyvehicles are available for business use; Both the ability to work independently and as a teammember;Excellent communication skills both verbal and written;andThe ability to be organized and have good timemanagement skills.The COVID-19vaccine is required or properly approved exemption consistent withapplicable laws. Benefits $20.06/hr, $41,724.80/annual equivalent. 40 hours per week.Monday-Friday.DAY TIMEHOURS AND NO ON-CALL.
