Job Description Job Summary: The Coordinator - Cancer Peer Education Program organizes and coordinates activities and work flow for the program. Responsibilities: Contributes to the development, implementation and evaluation of cancer programs, services and innovations. Develops, implements and maintains quality improvement and compliance activities. Develops and facilitates activities that will raise community awareness of cancer prevention, detection and treatment options. Develops and promotes strategic and educational initiatives designed to increase the quality and breadth of cancer-related patient/family support and enhance the quality of patient care. This position directs the overall operation of the project, with responsibility for overseeing the implementation of project activities through coordination with other agencies, developing materials, providing in-service education and training, and conducting meetings. Designs and directs the collection, tabulation, and interpretation of required data.This position is responsible for overall program evaluation, and is the responsible authority for ensuring necessary reports are submitted to the NYSDOH/HRI. Communicates effectively with internal and external parties to insure program goals are achieved using appropriate, accurate and non-judgmental language; asking open ended questions to solicit client information and give positive reinforcement; paraphrasing (reframe) what clients say to ensure a mutual understanding; and speaking and present information effectively to small and large groups of clients using written and visual materials, when appropriate. Identifies community service resources, and builds and maintains relationships with networks of community resources.Shares knowledge of community resources for health care, social services and additional support services, providing referrals to clients to health care and social services.Assists clients with accessing services and other resources, providing additional support and follow-up as needed. Promotes appropriate health information within the community by identifying key community leaders and organizations, as well as other community characteristics, to be able to find and engage target population.Educates community leaders on the importance of screening and accessing services to help promote cancer screening in their communities.Employs effective outreach methods and strategies to reach target populations, conveying health information in appropriate and accessible ways while motivating clients to engage in behavior change and access health services. Understands and respects cultural norms (such as language, communication styles, health beliefs and values, religion, attitudes toward government agencies, etc.) of the communities being served. Responsible for program staff effectiveness including appropriate oversight and performance management. Qualifications Education & Work Experience: HS or Equivalent and 5 -7 years experience in community outreach required. High School Diploma or Equivalent required-Associate's Degree preferred. 2 years of community outreach required. Prior work in a medical setting and medical terminology preferred. LPN preferred. Associates level degree preferred.
